Vizavance, formally known as Prevent Blindness Oklahoma, will be doing vision screenings for PK-6th gades on Monday, October 15th and 16th. If you DO NOT want your child to be screened, please print the opt-out form from the link below, sign and send it to the school nurse as soon as possible.
